| 目的 探讨支气管肺泡灌洗治疗对老年重症肺炎患者临床效果及SaO2、PaO2、PaCO2水平的影响。方法 选取2018 年9 月至2020 年9 月于我院的老年重症肺炎患者114 例，随机分为两组（对照组和研究组），各57 例。对照组应用常规治疗，研究组在此基础上应用支气管肺泡灌洗治疗。观察对比两组治疗前、后的炎症因子变化情况和动脉血气情况及治疗效果。结果 研究组治疗后WBC（7.8±2.6）×109/L、PCT（23.2±2.1）ng/mL、hs-CRP（4.5±2.1）mg/L 低于对照组（10.2±3.1）×109/L、（31.3±6.2）ng/mL、（7.1±3.2）mg/L（P＜0.05）；研究组治疗效果（94.8%）高于对照组（82.4%）（P＜0.05）；研究组治疗后SaO2（89.6±5.2）%、PaO2（84.2±7.1）mmHg 水平高于对照组（77.6±4.3）%、（77.1±5.2）mmHg，PaCO2水平（41.8±2.1）mmHg 低于对照组（50.5±3.7）mmHg（P＜0.05）。结论 老年重症肺炎患者应用支气管肺泡灌洗治疗效果理想，可以降低机体的炎症因子指标，改善动脉血气情况，提高治疗效果。 |

| Objective To investigate the effect of bronchoalveolar lavage treatment on the clinical effect and the levels of SaO2,PaO2,and PaCO2in elderly patients with severe pneumonia.Methods A total of 114 elderly patients with severe pneumonia in our hospital from September 2018 to September 2020 were selected and randomly divided into two groups (control group and study group),with 57 cases in each group.The control group was treated with conventional treatment,and the study group was treated with bronchoalveolar lavage on this basis.The changes of inflammatory factors and arterial blood gas before and after treatment,and the treatment effect in the two groups were compared.Results After treatment,WBC (7.8±2.6)×109/L,PCT (23.2±2.1) ng/mL,hs-CRP (4.5±2.1) mg/L in the study group were lower than those (10.2±3.1)×109/L,(31.3±6.2) ng/mL,(7.1±3.2) mg/L in the control group (P＜0.05).The treatment effect of the study group (94.8%) was higher than that (82.4%) of the control group (P＜0.05).The SaO2(89.6±5.2)%,PaO2(84.2±7.1) mmHg levels of the study group after treatment were higher than (77.6±4.3)%,(77.1±5.2) mmHg of the control group.The PaCO2level (41.8±2.1) mmHg in the study group was lower than (50.5±3.7) mmHg in the control group (P＜0.05).Conclusion The bronchoalveolar lavage in elderly patients with severe pneumonia has an ideal effect,which can reduce the body′s inflammatory factors,improve arterial blood gas,and improve the therapeutic effect. |
