      目的研究尿胱抑素C(Cystatin C,CysC)和α1-酸性糖蛋白(Alpha-1-acid glycoprotein,AAG)在早期糖尿病肾病诊断中的价值。方法 选择2017 年6 月至2018 年6 月上海市浦东新区周浦医院内分泌科住院的2 型糖尿病患者130 例,根据尿微量白蛋白(Urine microalbumin,U-mALB)的不同分为单纯糖尿病组(对照组)和微量白蛋白尿组(试验组)。采用酶联免疫吸附法(ABC-ELISA)检测尿CysC、尿AAG,比较各组尿CysC 和AAG 水平的差异,分析尿CysC、尿AAG 与尿U-mALB 的相关性。结果 对照组与试验组的尿CysC 分别为(1.026±0.673)mg/L和(1.556±0.846)mg/L,尿AAG 分别为(0.321±0.107)mg/L 和(0.368±0.142)mg/L,两组比较,差异有统计学意义(P<0.05);Pearson 相关分析显示,所有入组的糖尿病患者尿CysC(r=0.434,P=0.000)和尿AAG(r=0.267,P=0.002)与尿U-mALB 呈正相关;联合检测尿CysC 和尿AAG 的灵敏度、特异度、阳性预测值、阴性预测值均明显高于任一单项指标。结论检测尿CysC 和尿AAG 可反映糖尿病患者早期肾损伤情况,联合检测对于诊断早期糖尿病肾病具有较高的临床价值。

      Objective To study the value of urinary cystatin C(CysC) and Alpha-1-acid glycoprotein(AAG) in the early diagnosis of diabetic nephropathy.Methods A total of 130 patients with type 2 diabetes in the department of endocrinology of Zhoupu Hospital,Pudong New Area in Shanghai City from June 2017 to June 2018 were enrolled.According to the difference of urine microalbumin(U-mALB),they were divided into simple diabetes group(control group)and microalbuminuria group (experimental group).Urine CysC and urine AAG were detected by enzyme-linked immunosorbent assay(ABC-ELISA),and the differences in urine CysC and AAG levels between the groups were compared and analyzed.The correlation between urine CysC,urine AAG,and urine U-mALB was analyzed.Results The urine CysC of the control group and the experimental group were(1.026±0.673)mg/L and(1.556±0.846)mg/L,and AAG of the two groups were(0.321±0.107)mg/L and(0.368±0.142) mg/L,the differences were statistically significant(P<0.05).Pearson related analysis found that urine CysC(r=0.434,P=0.000) and AAG(r=0.267,P=0.002) of all enrolled diabetic patients were positively correlated with urine U-mALB;the sensitivity,specificity,positive predictive value,and negative predictive value of combined detection of urine CysC and AAG were significantly higher than those of any single index.Conclusion The detection of urine CysC and AAG can reflect the early renal damage of diabetes,and the combined detection has high clinical value for the diagnosis of early diabetic nephropathy.
